Our office staff will be glad to assist you with your insurance coverage. When you first visit our office, bring your current insurance card with you. If you change insurance companies or join another employer’s plan, please be sure to let our insurance department know. If an insurance referral is necessary, please bring the completed form with you to your appointment.

If you don’t see your insurance plan on the list, call us at 843.815.6468. We do not accept HMO plans, Advicare Medicaid, Blue Choice, Consumer’s Choice, Humana PPO, and Wellcare Medicaid.
